Medication scheduling
A nurse keeps the chart. A carer records the dose on the round.
Open Medications on the web. For each medicine add:
- Name and dose as on the pharmacy label
- Times of day (morning, midday, evening, bedtime - or custom)
- PRN / as-needed rules when relevant: indication, max dose, max in 24 hours, notes
After a hospital discharge or a new supply, a nurse re-checks the chart the same day.
Carers record given, missed, or refused on the round. Relatives do not record doses.
